Alzheimer's and Dementia Person Centered Symptom Tracker

Symptoms of Alzheimer’s/dementia can vary with each individual and at each phase of the disease. Biography Based Care’s Alzheimer’s and Dementia Symptoms Tracker helps caregivers articulate the types and frequency of the top 50 most commonly reported symptoms of dementia that loved one’s may be experiencing . This checklist, updated every month or when you notice significant changes can be shared with your physician, medical and other care providers. Recording the changes over time will help all of you to better see and track the progression of the disease in your loved one so that treatment options may be better customized to your loved one’s specific situation.

Reduce the Risk of Wandering Safety Checklist_Page_1

We so often hear from caregivers after a wandering episode that they NEVER thought that their loved one would wander, that their loved one had never done anything like that before. However, the Alzheimer’s Association estimates that 60% of those with dementia will wander at some point in  the disease process. This checklist is designed to help you identify which safety measures you and your physician believe would best help in your situation.  We recommend that you detach this checklist and take it with you to your physician appointments and to the hardware/medical supply stores as a guide. Though it is not possible to prevent the risk of wandering in people with Alzheimer’s/ Dementia. Caregivers may however, be better able to reduce the incidents and be better able to respond to this risky behavior by having a specific plan in place based on your loved one’s individual needs.

Website Version of Care Needs Calculator

Each family’s experience with Alzheimer’s/Dementia is different. Not only is the journey of those living with the disease specific to them, but also each caregiver’s experience is as unique as their skill set, physical ability and energy level.

The first step is to determine how much time you spend helping your loved one with specific tasks.  Secondly, what kind of help are you providing to support them in successfully completing their activities of daily living (cueing or total assistance for example). The Care Needs Calculator helps readers objectively determine these two things.

The Care Needs Calculator is intended to give general information to help support readers and their health care team in identifying how best to meet their loved one’s changing needs at different stages in the disease process. Many caregivers have reported using this tool to assist them in determining when more help at home/or placement outside the home may be needed.
